Adjustable Door Hinge -  Guide Hinge For Left Hand and Right Hand Doors - Horizontal Adjustment
Hinge is Steel, non handed and Includes Screws.
Known to be used on Windsor and Superior doors and is interchangeable with Hoppe Columbus 2-D Hinge part # S4002 (Part Number S4002 is a discontinued hinge and S4004 is its replacement).
